Interim HealthCare is seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to join our dedicated hospice team. This position focuses exclusively on conducting hospice admissions—you’ll be the clinical face of our program, guiding patients and families through the first step of their hospice journey with care, clarity, and compassion.

Schedule:
Work 4 days per week, 10-hour shifts on Monday, Tuesday, Wednesday, and Friday

Responsibilities

As a Hospice Admissions Registered Nurse (RN), here’s a big-picture view of what you’ll do:

  • Perform only admissions visits for hospice patients—no routine or follow-up case management required
  • Conduct comprehensive in-home assessments to determine hospice eligibility and patient/family needs
  • Initiate individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and the patient’s physician
  • Educate patients and families on the hospice philosophy, services, and symptom management
  • Coordinate care and services across the healthcare team to ensure seamless support
  • Support pain and symptom control with expert nursing care and ongoing assessments
  • Complete timely documentation and participate in quality improvement initiatives

Skills & Qualifications

A few must-haves for Hospice Admissions Registered Nurses (RN):
  • Graduate of an accredited Registered Nurse (RN) program
  • Active, unrestricted Registered Nurse (RN) license in the State of Georgia or compact multi-state license
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
  • One (1) year of hospice care experience preferred.
