We are looking for a Hospice Intake Coordinator to manage the admissions process, working closely with clinical staff, patients, referral sources, and the business development team. This role ensures timely communication and coordination of patient care, following agency standards. In this role, you’ll serve as a patient care advisor and liaison between Hospice Intake Coordinators, Business Development Team liaisons clinical staff, physicians, and patients.

Responsibilities
As a Hospice Intake Nurse - LPN, here’s a big-picture view of what you’ll do:
- Coordinate patient admissions, ensuring a smooth and timely process.
- Contact patients and families within an hour of referral, and aim to complete admissions the same day.
- Work with clinical and management staff, referral sources, and patients to collect health information and arrange assessments.
- Handle all patient care orders and documentation via the electronic medical record system.
- Collaborate with the Business Development team to build relationships with referral sources and support growth strategies.
- Provide education about hospice services to patients and families, and clarify treatment plans as needed.
- Coordinate necessary services like medical equipment and treatments for patients.
- Handle patient concerns and follow up on pending referrals.
- Ensure compliance with agency standards, participate in quality improvement activities, and assist with new employee training.

A few must-haves for Hospice Intake Nurses - LPN:
- Graduate of an accredited school of nursing and licensed to practice as an LPN in SC.
- 3 years clinical hospice experience in hospice is preferred.
- Strong customer service experience.
- Hospice sales experience preferred.
