Responsibilities
As a Hospice Licensed Practical Nurse (LPN), here’s a big-picture view of what you’ll do:
- Provide compassionate, patient-centered nursing care to hospice patients in their homes or place of residence, under the supervision of a Registered Nurse (RN).
- Support patients with terminal illness by focusing on comfort, symptom management, and quality of life.
- Collaborate closely with physicians, RNs, CNAs, social workers, chaplains, and other members of the interdisciplinary hospice team.
- Perform skilled nursing tasks including taking vital signs, conducting focused assessments, administering medications as ordered, and monitoring patient status.
- Accurately document patient care, observations, and changes in condition, and promptly communicate updates to the RN case manager.
- Assist the RN in implementing the individualized plan of care and reinforcing patient and caregiver education.
- Educate patients and families on medications, symptom control, disease progression, and comfort measures.
- Provide emotional support to patients and families, fostering dignity, respect, and compassionate end-of-life care.
- Follow all hospice policies, procedures, and regulatory guidelines to ensure safe, ethical, and compliant care delivery.

- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the South Carolina.
- At least one-year or 1200 hours of experience as an LPN/LVN.
